Title: Can Data Make a Dent in Addressing Social Determinants of Health?

Description: Are you buried in data but unsure how to identify upstream determinants that will have a significant impact on your community’s health?  Join Candice Davenport, Robin Vlamis, and Linda Brown, Past and current Presidents of NJ SOPHE, who will be sharing the development of a new model for synthesizing various public health data sources. They will use their Community Health Data Alignment Model to pinpoint relevant public health issues in a community that can turn the tide on social determinants of health and a community's quality of life. 

 

Webinar Objectives:

  1. To define the 'upstream'/ 'downstream' model when discussing health programs and interventions
  2. To illustrate how efforts to address social determinants of health can have positive ramifications on health outcomes and lead to measurable change
  3. To demonstrate how to synthesize various data sources using the new 'Community Health Data Alignment Model' (CoHDA) that can assist in identifying tangible and specific issues and underlying social determinants of health in a targeted community
